70 % przyczyn poronień to wady genetyczne zarodka. Nie wykonuje się
badań genetycznych resztek po poronieniu bo i po co.
A co do CMV. W żadnym kraju nie jest o badanie screeningowe ani
obligatoryjne w ciąży .
Pozwolę sobie na cytat z podręcznika Gabbe'a ( "Obsatetrics - normal and
problem pregnancies"):
At the present time, a vaccine for CMV is not available. Antiviral
agents such as ganciclovir and foscarnet have moderate activity against
CMV, but their use is limited primarily to treatment of severe
infections in immunocompromised patients. Accordingly,
obstetriciangynecologists should focus most of their attention on
educating patients about preventive measures.
One of the most important interventions is helping patients understand
that CMV infection can be a sexually transmitted disease and that sexual
promiscuity significantly increases an individual's risk of acquiring
the infection. Individuals who have multiple sexual partners should be
counseled that latex condoms provide an effective barrier to
transmission of CMV.[96] Another important intervention is educating
health care workers, day-care workers, elementary school teachers, and
mothers of young children about the importance of simple infection
control measures such as handwashing and proper cleansing of
environmental surfaces. Obstetricians and pediatricians must be
consistently aware of the importance of transfusing only CMV-free blood
products to fetuses, neonates, pregnant women, and immunocompromised
patients, and screening potential donors
of organs and semen for CMV infection.[71] Finally, health care workers
must adhere to the principles of universal precautions when treating
patients and handling potentially infected body fluids.[84]
For several reasons, routine prenatal screening for CMV infection is not
recommended. First, laboratory resources could be overwhelmed if all
pregnant women were screened. Second, if laboratories do not ensure a
high level of quality control, the interpretation of serologic tests may
be confusing and may lead to incorrect and irreversible interventions
such as pregnancy termination. Third, neither antiviral chemotherapy nor
immunoprophylaxis is available to protect the fetus or neonate.
Accordingly, screening should be limited to women who have symptoms
suggestive of acute CMV infection, who have had definite occupational
exposure to CMV, or who are immunocompromised.
Notabene rutynowy screening w kierunku toxo również nie jest zalecany.
Regularnie trafiają sie pacjetki wpadające do gabinetu z przerażeniem w
oczach : doktorze moje dziecko będzie miało toxoplazmozę. A w wynikach
wzrost IgG z 15 na 18 .
Może i to okrutne co napiszę ale jedno poronienie to jeszcze nie powód
do rozpaczy nawet jeżeli jest to wyczekiwana ciąża . po 2 ( niektórzy
uważają ,że po 3) poronieniach należy wykonac badania w kierunku chorób
autoimmunologicznych, HSG,, badania hormonalne, genetyczne.
A jedna dobra wiadomośc , którą niesie przebycie poronienia: mogę mieć
dziecko! to fatalne zdarzenie wyklucza część diagnostyki, która muszą
przejśc pary nie mogące w ogóle zjść w ciążę.
